Schedule / Job TypeContract | Full-Time | Days | Monday through Friday, 8:00 AM - 5:00 PM | Onsite / Field-Based | No on-call or weekend requirements
Key Responsibilities- Provide physical therapy assistant services to patients in a home health setting across an assigned service territory
- Carry out treatment plans established by a supervising Physical TherapistComplete approximately 6 patient visits per day in accordance with productivity expectations
- Document patient progress and clinical notes accurately and in a timely manner
- Communicate effectively with the supervising Physical Therapist regarding patient status and plan of care updates
Required Qualifications- Active Physical Therapist Assistant (PTA) license in the state of California
- Minimum of one year of home health experience
- Ability to travel to patient homes within the designated service area
- Valid driver's license and reliable transportation
Preferred Experience / Skills- Experience with HCHB (Homecare Homebase) electronic medical records system
- Familiarity with the service territory including the surrounding communities in the Inland Empire region
