Schedule / Job TypeContract | Full-Time | Days (Monday–Friday, 8:00 AM–5:00 PM) | No On-Call or Weekend Requirements
Key Responsibilities- Deliver skilled physical therapy services to patients in a home health setting under the supervision of a licensed Physical Therapist
- Manage a caseload of approximately 6 patient visits per day across assigned service territories
- Implement and progress individualized treatment plans in accordance with the supervising Physical Therapist's plan of care
- Document patient progress, treatment notes, and visit records accurately and in a timely manner
- Communicate effectively with the care team regarding patient status and changes in condition
Required Qualifications- Active Physical Therapist Assistant (PTA) license in the state of California
- Minimum of one year of home health experience
- Ability to travel to patient homes within the assigned service area
- Strong documentation and communication skills
Preferred Experience / Skills- Experience with HCHB (Homecare Homebase) EMR software
- Familiarity with home health workflows and regulatory requirements in California
