Key Responsibilities- Provide physical therapy assistant services to patients in a home health setting
- Carry out treatment plans established by the supervising Physical Therapist< li>Meet minimum productivity expectations of 32 points per week
- Accurately document patient care using HCHB electronic medical records system
- Complete OASIS assessments as required within the home health setting
Required Qualifications- Active Physical Therapist Assistant (PTA) license
- Minimum of 12 months of home health experience within the past 3 years
- Proficiency with HCHB EMR system
- Experience with OASIS documentation
Preferred Experience / Skills- Strong time management and self-directed work habits suited for a home health environment
- Familiarity with home health productivity and point-based scheduling systems
