Physical Therapy Assistant (PTA)

Schedule / Job TypeContract | Days | Monday–Friday | 10-Week Assignment
Key Responsibilities- Provide physical therapy assistant services to patients in a home health setting within an assigned territory
- Complete approximately 30 patient visits per week across the assigned service area
- Utilize HCHB electronic medical records system to document patient care and progress
- Collaborate with the supervising physical therapist and care team to implement individualized treatment plans
- Maintain compliance with home health regulations and organizational standards throughout the contract period
Required Qualifications- Active Physical Therapist Assistant (PTA) license
- Minimum of 12 months of home health experience within the past 3 years
- Proficiency with HCHB (HomeCare HomeBase) EMR system
- Ability to manage a caseload of approximately 30 visits per week
- No on-call requirements
Preferred Experience / Skills- Familiarity with urban home health service territories
- Strong time management and independent scheduling skills in a field-based care environment
