Location: Jeffersonville, IN

Pay: $2,639.20 weekly

Shifts: M-F 8-5 and a minimum of one weekend a month on call

This role focuses on providing quality physical therapy evaluation and treatment to patients in a supportive and collaborative environment. The position involves working onsite and leveraging specialized skills in Home Health, HCHB, and OASIS to enhance patient outcomes.

Key Responsibilities of the Physical Therapist
  • Perform physical therapy evaluation and treatment based on physician orders/referrals
  • Complete comprehensive evaluations using appropriate tools and procedures
  • Review patient records for diagnosis, history, precautions, and contraindications
  • Establish measurable goals and develop treatment plans to maximize patient independence/function
  • Regularly reassess patient progress and modify treatment plans as needed
  • Communicate patient needs and progress with the treatment team and involved parties
  • Consult with medical providers regarding patient evaluation, treatment, progress, and discharge
  • Recommend and secure adaptive equipment to enhance the rehabilitation program
  • Maintain and submit required documentation in compliance with various regulations
  • Supervise Physical Therapist Assistants and Aides in patient care activities
  • Participate in staff development programs and obtain continuing education

Requirements of the Physical Therapist
  • Valid Indiana license
  • Experience in Home Health, HCHB, and OASIS
