This role specializes in providing home health services within various counties, including Butler, Crenshaw, Lowndes, and Covington. As a Physical Therapist, you will be responsible for evaluating and treating patients, tailoring your approach to ensure optimal independence and function.

Key Responsibilities of the Physical Therapist
  • Perform physical therapy evaluation and treatment based on physician orders/referrals
  • Complete comprehensive evaluations using appropriate tools and procedures
  • Establish measurable goals and develop treatment plans for patient independence and function
  • Regularly reassess treatment effectiveness and patient progress
  • Communicate patient needs and progress to the treatment team and family members
  • Consult with medical providers about patient evaluation, treatment, and discharge
  • Recommend and assist in securing adaptive equipment for rehabilitation
  • Maintain and submit necessary documentation in compliance with regulations
  • Supervise Physical Therapist Assistants and Aides in patient care activities
  • Participate in internal staff development programs and maintain clinical practice skills

Requirements of the Physical Therapist
  • Active Physical Therapist license
  • Experience in Home Health required
  • HCHB experience preferred but not required
