Seven Healthcare is seeking a motivated and skilled Physical Therapist for a contract opportunity in Winston Salem, NC. This position requires expertise in home health physical therapy, utilizing tools such as Oasis and HCHB, set in an engaging and supportive work environment.

Key Responsibilities of the Physical Therapist
  • Perform physical therapy evaluation and treatment based on physician orders/referrals
  • Complete comprehensive evaluations using appropriate tools and procedures
  • Review patient records for diagnosis, medical history, precautions, and contraindications
  • Establish measurable goals and develop treatment plans
  • Re-assess treatment plans regularly and modify as needed
  • Communicate patient needs and progress with the treatment team
  • Consult with medical providers regarding evaluations, treatment, and patient progress
  • Recommend and assist in securing special adaptive equipment for patients
  • Maintain and submit documentation in required formats and timelines
  • Supervise Physical Therapist Assistants and Aides
  • Participate in and conduct staff development programs and meetings

Requirements of the Physical Therapist
  • Active NC PT license
  • Experience in Home Health PT
  • Proficiency with Oasis and HCHB
