The role involves providing comprehensive physical therapy care within a home setting, supporting the Monroe County team. Key Responsibilities of the Physical Therapist
  • Develop and revise treatment and discharge plans in conjunction with the patient, family, and care team
  • Administer physical therapy assessment and treatments in a home setting
  • Supervise Physical Therapy Assistants (PTA) to ensure compliance with care plans
  • Consult with patients and families to promote understanding and implementation of home therapy programs
  • Ensure the completion of clinical records and compliance with regulatory requirements
  • Participate in patient care conferences and interdisciplinary team meetings
  • Maintain a professional and supportive environment for patients and their families

Requirements of the Physical Therapist
  • B.S., M.S., or D.P.T. degree in Physical Therapy
  • Current New York State License in Physical Therapy
  • BLS (AHA only)
  • Minimum of one-year experience in a long-term care or home health setting
  • Recent Home Health experience (within the last year)
  • Experience with Oasis documentation
  • Strong interpersonal and communication skills
