Key Responsibilities of the Physical Therapist
  • Perform physical therapy evaluations and treatments based on physician orders/referrals
  • Complete comprehensive evaluations using appropriate assessment tools
  • Establish measurable goals and develop treatment plans for patient independence/function
  • Regularly reassess and modify treatment plans as necessary
  • Communicate patient needs and progress with the treatment team and family
  • Consult with medical providers about patient progress and discharge
  • Assist in procuring and incorporating adaptive equipment to enhance rehabilitation
  • Maintain and submit required documentation per regulations
  • Supervise Physical Therapist Assistants and Aides
  • Participate in staff development programs and continue education

Requirements of the Physical Therapist
  • Licensed NC PT or compact license
  • Home health experience
  • Experience with HCHB and OASIS
