Job Title: PTA Home Health Profession: Therapy/Rehabilitation Specialty: PTA Home Health Duration: 13 Weeks Shift: Day Hours per Shift: 8 Hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Recent Home Health experience Description: Home visits required. Periodic office meetings. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day is 5-6. Contractor will be assigned between 5-10 patients per day that equate to roughly 6-8 points. Each type of visit and activity (documentation, travel, etc.) holds a different point value. In a situation where the contractor is unable to meet their financial needs due to low patient census, please reach out to discuss floating to a nearby clinic to pick up hours. Types of cases include Start of Care and Follow-Ups. First-timers are not accepted. Weekend requirements will vary per location and profession. Traveling may be required up to 75 miles from home clinic. Mileage will be reimbursed at the IRS standard rate. Commute from the first and last patient are not billable. Locals are accepted at the same rate. Pending licenses are not accepted, must have license in hand unless obtaining it takes a few days. This contractor is expected to travel variable distances for patient coverage. Estimated total number of miles per week is 200. Weekly units of productivity expectations are 32. Proficiency in HomeCare HomeBase EMR is required. All travelers must be proficient in the EMR system and have used it within the last 6 months. All documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation will impact billing and follow-up patient care. Expectations include accepting every patient on their schedule regardless of distance. No guaranteed hours with this contract. Clinician must hit weekly productivity points or face potential adjustments. Opportunity to float to another nearby clinic will be available if needed.
