Job Title: Therapy PTA Home Health Profession: Therapy/Rehabilitation Specialty: PTA Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year of Adult Home Health Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Must have reliable transportation. Must have a current driver's license. Must have agency-required liability insurance. Must be proficient in OASIS and have used it within the last 6 months. Must be proficient in HomeCare HomeBase EMR and have used it within the last 6 months. Description: This position involves home visits for therapy services. Periodic office meetings will also take place. The contractor will be assigned between 5-10 patients per day. Average number of patients per day is 5-6. Each visit and activity, including documentation and travel, has a different point value. Weekly productivity points expectation is 30. Traveling up to 75 miles from the home clinic is required. Mileage will be reimbursed at the IRS standard rate. The commute to and from the first and last patient is not billable. Local candidates are accepted with the same rate. All documentation must be completed within 24 hours of the appointment. Clinicians must accept every patient on their schedule. Declining patients may result in a cancelation. There are no guaranteed hours for this contract. If the clinician is not receiving enough patients, they may have the opportunity to cover at a nearby clinic to pick up additional patients. Weekend requirements may vary per location and profession, and should be discussed during the interview. On-call expectations will vary per job and location. Experience in Home Health is required. The contractor will be expected to manage patient assessments, discharge evaluations, and care plans. Strong time management skills are essential for success in this role.
