Description: This position is responsible for the evaluation, planning, administration, and supervision of Physical Therapy treatment in accordance with physician referrals, state and national standards, regulatory agencies, and established departmental policies and procedures. This position demonstrates the clinical competence and knowledge necessary to provide appropriate treatment based on patient age. The role includes evaluating a patient's functional abilities and limitations. It determines if intervention is needed and establishes a plan of care and treatment goals. Comprehensive evaluations must be completed within the established timeframe. Key limiting factors are identified and prioritized, and treatment goals must be functional, measurable, and patient-related. Collaboration with patient and family/caregiver is essential in goal setting. Initiates discharge planning and recommends modifications to referring orders. Implementing individualized treatment plans that consider the patient's physical, social, spiritual, and cultural needs is critical. Skilled treatment must be provided in a safe manner. Ongoing effectiveness of treatment is assessed, and plans are modified as needed. Patient and family/caregiver involvement in the treatment and decision-making process is necessary. Ongoing education should be provided to physicians, nurses, and interdisciplinary teams. Documentation must adhere to professional practice guidelines set by regulatory agencies. All documentation should be completed legibly within established timeframes. Document evidence-based and individualized treatment techniques that reflect patient progress or lack thereof. Reassessment of treatment goals is done according to departmental policies. Effective communication of pertinent patient information is critical for best follow-through of care. Evaluation and treatment results, as well as patient progress, should be communicated clearly and timely to physicians, nurses, patients, and families. Responsibility for contacting physicians for problem-solving is required to ensure optimal patient outcomes. Treatment rationale must be defensible with standard practices or current literature. Flexibility in assignments related to caseload, scheduling, and staffing shifts is expected. Taking initiative to assist in other patient care areas while accepting responsibility for all patient care needs is necessary. Demonstrating appropriate supervision and delegation of personnel ensures quality patient care. Supervision must comply with departmental policies and regulatory agencies. Participation in the training and orientation of new personnel is required. Assuring competency of personnel prior to delegation of tasks is essential. Attending required departmental meetings and participating in process improvement activities is expected. Meeting productivity standards set by the department is necessary. This role involves interactions with various staff levels across departments, physicians, patients, and families. Interactions with external contacts, such as community providers, concerning patient healthcare is also essential. This position requires strong collaboration and communication skills to ensure the best outcomes for patients.
