Job Title: Physical Therapy Assistant Profession: Therapy/Rehabilitation Specialty: PTA Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Recent Home Health experience Description: Home visits for therapy PTA in a home health setting. Periodic office meetings required. Must have reliable transportation. Current driver's license and agency-required liability insurance are needed. Average number of patients per day: 5-6. Contractor will be assigned between 5-10 patients per day. Points per week to be achieved: 30. Each type of visit and activity holds a different point value. Types of cases include Start of Care and Follow Ups. If there is a situation where the contractor is unable to meet financial needs due to low patient census, please reach out to the manager to discuss options. Must be willing to travel up to 75 miles from home clinic. Mileage reimbursed at the IRS Standard rate. Traveler is compensated from patient home to patient home. The commute to and from the first and last patient is not billable. Local candidates are accepted at the same rate. No pending license accepted—must have license in hand unless it takes a few days to obtain. Must confirm awareness of home health responsibilities. Clinician must have a vehicle, driver's license, and auto insurance. Comfort with drives that may exceed 20 minutes between patients is necessary. Clinician should be prepared to accept every patient on their schedule. Documentation must be completed within 24 hours of the appointment. There are no guaranteed hours with this contract. Clinician must meet weekly productivity points, which equate to a standard work week. If weekly patient load is insufficient, options to float to a nearby location will be available.
