Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active California PTA license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• At least one year of home health (HH) experience required.
• Experience with HCHB electronic charting preferred but not required.

Preferred Qualifications

• Prior experience working with home health patient populations and care plans.
• Familiarity with documentation and reimbursement processes in home health.

Assignment Details

• Shift hours: 8:00 AM to 5:00 PM.
• Work schedule: Monday through Friday, 8-hour shifts.
• Service area includes multiple neighboring cities such as Yorba Linda, Orange, Anaheim Hills, and Santa Ana; candidate must be able to travel within the service area as assigned.
• No on-call or weekend requirements.
