Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Minimum 1 year of home health experience with a Medicare certified facility
• HCHB experience required

Assignment Details

• Schedule: Monday through Friday, 8:00 AM to 5:00 PM
• Weekend or on-call requirements apply
• Coverage area includes multiple communities throughout the surrounding region
