Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active California PTA license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Minimum 1 year of home health experience with a Medicare-certified facility (client requirement).
• Experience using HCHB EMR required (client requirement).
• Productivity expectation: approximately 30 points per week (client requirement).

• Shift: Days.
• Schedule: Monday–Friday, 8:00 AM–5:00 PM.
• Typical shift length: 8 hours.
• Guaranteed hours per week: 40.
• On-call or weekend requirement: 2–3 weekend shifts per month.
• Territory includes Capistrano Beach, Rancho Mission Viejo, Ladera Ranch, San Clemente, and San Juan Capistrano; travel between patient homes within this territory is required.
