Position Summary:
The Physical Therapist Assistant (PTA) provides skilled physical therapy services under the supervision and direction of a licensed Physical Therapist (PT). The PTA implements treatment plans to improve residents mobility, strength, balance, and overall functional ability, promoting independence and quality of life within a skilled nursing facility setting.

Key Responsibilities:
  • Carry out physical therapy treatment plans developed by the Physical Therapist to address residents' mobility and functional needs.
  • Deliver direct therapy services, including therapeutic exercises, gait training, and balance activities, in a safe and effective manner.
  • Observe and report resident responses to interventions, including progress and changes in condition, to the supervising PT.
  • Assist with resident assessments and contribute to documentation and care planning as appropriate.
  • Educate residents and caregivers on exercises and techniques to promote safety and carryover outside therapy sessions.
  • Maintain accurate, timely, and compliant documentation in the electronic medical record system.
  • Ensure proper use and care of therapy equipment and assistive devices.
  • Participate in team meetings and collaborate with interdisciplinary staff to support coordinated resident care.
  • Adhere to all state, federal, and facility regulations and professional standards of practice.
  • Uphold ethical standards and maintain licensure and continuing education requirements.
Qualifications:
  • Graduate of an accredited Physical Therapist Assistant program.
  • Current PTA license in the state of practice.
  • Prior experience in SNF or geriatric setting preferred.
  • Strong communication, clinical judgment, and documentation skills.
  • Ability to work effectively as part of a rehabilitation team under the supervision of a licensed PT.
