Job Title: Physical Therapist Assistant Profession: Physical Therapist Assistant (PTA) Specialty: Long Term Care (LTC) Duration: 13 weeks Shift: Days Hours per Shift: 8 hours M-F, times are flexible as long as consistent and communicated Experience: 1+ years of PTA experience, LTC or Rehab Experience License: Active PTA license Certifications: N/A Must-Have: Skills checklist within 1 year, 0-1 reference at time of submission Description: Utilizes evidence-based practice to support clinical interventions. Provides rehabilitative, skilled, and medically necessary treatment intervention to patients with physical functioning disorders. Administers various procedures as part of the rehabilitation plan under the supervision and direction of a Physical Therapist. Assesses patient progress and communicates with the supervising Physical Therapist to adjust treatments accordingly. Reports effectiveness of treatment outcomes to the supervising Physical Therapist. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with the supervisor and other health team members regarding patient progress, barriers, and treatment plans. Provides input to the supervisor for discharge summary of services provided. Documents all regulatory and clinically required information in the electronic documentation system. Maintains patient records according to company policy. Accurately records treatment minutes for services provided. Participates in required meetings including patient care conferences and rehabilitation meetings. Engages in facility and company required training, in-services, and conferences. Works with the supervising Physical Therapist to obtain any necessary Durable Medical Equipment (DME) or supplies. Supervises and supports students under the guidance of the Physical Therapist. Adheres to practice acts, code of conduct, and ethics during professional practice as a Physical Therapist Assistant. Holds active state licensure when applicable.
