Minimum Qualifications
- Associate degree in Physical Therapy from an accredited educational program.
- Current, active PTA license in the state of Florida is required to start the assignment.
- PTA certification required.
- Home Health experience required.
- Experience with HCHB and PointClickCare (PointCare) documentation systems required.
- BLS certification from AHA or ARC may be required.
- Minimum education: Associates Degree.
- Preferred: prior travel home health experience and familiarity with reimbursement and payer guidelines.
Assignment Details
- Facility type: Home Health.
- Shift: Days.
- Shift hours: 0800–1600 (Monday through Friday schedule noted as 8:00 AM–4:30 PM in hiring manager guidance).
- On-call/weekend requirement: No.
- Coverage area: Saint Petersburg, FL.
- Floating: May be required to cover different patient caseloads within the agency service area.
- Documentation: Use of HCHB and PointClickCare required for patient records and visit tracking.

Role Summary and Responsibilities
- Provide skilled PTA services in patient homes under PT supervision, implementing established Plan of Care.
- Document patient response to treatment in HCHB/PointClickCare and communicate status changes to the interdisciplinary care team in accordance with agency policy and state regulations.
- Perform functional mobility training, therapeutic exercise, wound care support per plan of care, and patient/caregiver education to promote safety and independence.
- Coordinate visits, mileage between visits reimbursed per agency policy, and manage schedule within assigned coverage area.
