Job Summary (List Format):

- Provide skilled physical therapy services to residents under the supervision of a licensed Physical Therapist in a skilled nursing facility.
- Implement treatment plans to improve residents mobility, strength, balance, and overall functional ability.
- Deliver direct therapy services, including exercises, gait training, and balance activities, ensuring resident safety and effectiveness.
- Observe residents responses to therapy, reporting progress and changes to the supervising PT.
- Assist with resident assessments, documentation, and care planning as directed.
- Educate residents and caregivers on exercises and safety techniques for continued progress outside therapy sessions.
- Maintain accurate and compliant documentation in the electronic medical record system.
- Ensure proper use and maintenance of therapy equipment and assistive devices.
- Participate in interdisciplinary team meetings to support coordinated resident care.
- Adhere to all regulatory and professional standards, upholding ethical practices and licensure requirements.
- Work in a collaborative, fast-paced skilled nursing facility environment, requiring physical activity such as standing, lifting, and assisting residents.
