Job Summary (List Format):

- Provide skilled physical therapy services to residents under the supervision of a licensed Physical Therapist (PT).
- Implement treatment plans designed to improve residents mobility, strength, balance, and functional abilities.
- Deliver direct therapy services such as therapeutic exercises, gait training, and balance activities.
- Monitor and report resident progress and any changes in condition to the supervising PT.
- Assist in resident assessments and contribute to care planning and documentation.
- Educate residents and caregivers on safe exercise techniques and promote carryover outside therapy sessions.
- Maintain accurate and timely documentation in the electronic medical record system.
- Ensure proper use and maintenance of therapy equipment and assistive devices.
- Participate in interdisciplinary team meetings to support coordinated care.
- Adhere to regulatory requirements, professional standards, and ethical guidelines.
- Maintain active PTA licensure and meet continuing education requirements.
- Work primarily in a skilled nursing facility or long-term care setting, collaborating within a fast-paced, team-oriented environment.
