Minimum Qualifications

  • Associate degree in Physical Therapy from an accredited educational program.
  • Active Missouri PTA license required to start the assignment.
  • BLS certification required from American Heart Association or American Red Cross.
  • Minimum 12 months of home health experience within the past 3 years is required.
  • Experience with HCHB (HomeCare HomeBase) EMR and OASIS documentation is required.
  • Productivity expectation: must meet minimum productivity of 32 points per week (client-specific requirement).
  • Previous experience providing care in home health settings preferred.
