Minimum Qualifications

  • Associate degree in Physical Therapy from an accredited educational program.
  • Active State License is required to start the assignment.
  • BLS Certification may be required from AHA or ARC.
  • Must have at least 12 months of Home Health experience within the past 3 years (client requirement).
  • Experience with HCHB (HomeCare HomeBase) electronic medical record is required (client requirement).
  • Previous experience providing care in a home health setting is required.
  • Preferred: additional certifications or training in geriatric or orthopedic home health care.
