Minimum Qualifications

  • Associate degree in Physical Therapy from an accredited educational program.
  • Active Wisconsin PTA license required to start the assignment.
  • BLS certification required from the American Heart Association or American Red Cross.
  • Experience working in a skilled nursing facility or long-term care setting preferred.
  • Experience providing restorative or skilled rehab services to geriatric patients preferred.
  • Ability to follow treatment plans established by a supervising Physical Therapist.
  • Current immunizations and ability to pass facility background and health screenings as required.

  • Shift Name: Days.
  • Shift Details: Days 0830-1630.
  • Schedule Pattern: 5x8.
  • Floating: Candidate may be required to float to other facilities within the client network.
