Minimum Qualifications

• Associate degree in Physical Therapy from an accredited program.
• Active Connecticut PTA license required to start the assignment.
• Minimum 1 year of professional skilled nursing facility (SNF) experience required.
• BLS certification required from American Heart Association or American Red Cross.
• Willingness and ability to meet client productivity expectations (example: target productivity benchmarks applied during assignment).
• Compliance with Connecticut fingerprinting/background screening requirements.
• Preferred: experience with restorative nursing programs, electronic medical records used in SNF settings, and experience with common outpatient and long-term care therapeutic modalities.

Assignment Details

• Facility Type: Skilled Nursing Facility.
• Shift: Days.
• Shift Schedule: Monday through Friday.
• Shift Hours: 08:30–16:30.
• Guaranteed Hours: 35 hours per week.
• Assignment Length: 13-week contract.
• Meal Break: 30 minutes.
• Start: ASAP (on hold at this time).
• May be required to follow facility floating policies to cover other units within the same facility network.
