Minimum Qualifications

  • Associate degree in Physical Therapy from an accredited program.
  • Active Illinois PTA licensure required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Minimum education: Associate degree.
  • Client requirement: experience or ability to work in home health settings.
  • Candidate must be able to document visits electronically; familiarity with HCHB is preferred.


Preferred Qualifications

  • Home Health clinical experience preferred.
  • Experience using HCHB electronic documentation preferred.


Assignment Details

  • M-F schedule, 8:00 AM – 4:30 PM.
  • Coverage across the agency service area; travel between patient homes required.
  • Mileage reimbursed between visits from first patient to last patient.
