Minimum Qualifications

• Associate degree in Physical Therapy from an accredited program.
• Active Illinois PTA license required to start the assignment.
• BLS certification from AHA or ARC preferred.
• Experience working in a skilled nursing facility required.
• Experience with 5x8 day schedules and documentation in long-term care preferred.
• Ability to collaborate with interdisciplinary teams and provide restorative care.
