    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction
    • Conduct 60-minute evaluations and 30-minute follow-up visits in an outpatient setting
    • Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
    • Delegate and direct discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support effective return to community activities
    • Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned with appropriate supervision and evaluation
    • Adhere to discipline-specific state practice acts and regulatory requirements, and actively pursue professional development through continuing education and professional activities
    Required Qualifications
    • Active and unrestricted Physical Therapist license in good standing in the state of Colorado
    • Compliance with all state-mandated continuing education requirements
    • Ability to perform comprehensive patient evaluations and develop appropriate clinical treatment plans
    • Knowledge of and adherence to discipline-specific professional code of conduct and ethical standards
    • Experience with medical record documentation in compliance with state, federal, and payer guidelines
    Preferred Experience / Skills
    • Prior experience in an outpatient physical therapy setting
    • Experience supervising or mentoring physical therapy students
    • Familiarity with performance improvement methodologies and quality initiatives
