Key Responsibilities- Provide physical therapy services to patients in a home care setting
- Evaluate and assess patient conditions to develop individualized treatment plans
- Implement therapeutic interventions to improve patient mobility, strength, and function
- Document patient progress and treatment notes accurately within the EMR system
- Collaborate with interdisciplinary care teams to coordinate patient care
Required Qualifications- Active Physical Therapist license in the state of Ohio
- Experience in a home care setting
- Proficiency with Epic EMR
- Strong clinical assessment and documentation skills
Preferred Experience / Skills- Prior contract or travel therapy experience
- Strong communication and patient education skills
