    Key Responsibilities
    • Perform evaluative and therapeutic procedures under physician or accepted healthcare provider orders, observing all precautions, indications, and contraindications.
    • Evaluate residents per department protocols and establish individualized plans of care including functional goals, frequency and duration of services, and therapeutic modalities.
    • Monitor and progress treatment programs toward established functional goals, modifying plans of care as necessary.
    • Document all evaluative and therapeutic interactions with residents in the Electronic Medical Record in accordance with Medicare, Medicaid, and other reimbursement requirements.
    • Provide consultation to referring physicians and interdisciplinary care team members regarding plans of care and discharge needs; attend relevant team meetings.
    • Supervise assistant-level staff as appropriate, provide mentorship to new staff during orientation, and deliver in-service education to nursing and other departments.
    Required Qualifications
    • Bachelor's degree in Physical Therapy required.
    • Active Physical Therapist license in the applicable state.
    • Prior experience in a skilled nursing facility is required.
    • Strong documentation skills with working knowledge of Medicare and Medicaid reimbursement regulations.
    • Ability to lift up to 50 pounds; lifts above 50 pounds require a two-person assist.
    Preferred Experience / Skills
    • Master's or Doctorate degree in Physical Therapy preferred.
    • Basic Life Support (BLS) certification preferred.
    • Prior experience in a long-term care setting preferred.
    • Proficiency in Microsoft Office applications.
