Physical Therapist (PT) – Home Health

Schedule / Job TypeContract | Days | Monday – Friday, 8:00 AM – 5:00 PM | No weekend or holiday requirements
Key Responsibilities- Provide physical therapy services to home health and hospice patients in a home health setting
- Treat approximately 5–7 patients per day across an assigned geographic coverage area
- Evaluate and treat patients with diagnoses including post-surgical recovery, fall-related injuries, and related conditions
- Document patient care and progress using the HomeCare/HomeBase EMR system
- Work independently without on-site PT manager or supervisor oversight during shifts
Required Qualifications- Active North Dakota Physical Therapy License
- Current BLS certification
- Minimum 1 year of physical therapy experience
- Previous home health experience required
- Valid driver's license and current auto insurance required
Preferred Experience / Skills- Experience working with home health and hospice patient populations
- Familiarity with HomeCare/HomeBase EMR platform
