Job Overview:
We are currently seeking a dedicated and experienced Physical Therapist (PT) for a 14-week travel assignment at a reputable Skilled Nursing Facility in Vallejo, CA. This is a great opportunity to work in a collaborative environment while providing high-quality patient care.
Assignment Details:
- Shift: Day Shift
- Schedule: 8:00 AM – 4:30 PM
- Days: Flexible (Sunday through Saturday coverage)
- Guaranteed Hours: 35 hours/week
- Contract Duration: 14 weeks
Key Responsibilities:
- Evaluate and assess patients’ physical conditions
- Develop and implement individualized treatment plans
- Provide therapeutic exercises and rehabilitation programs
- Monitor patient progress and adjust treatments as needed
- Collaborate with interdisciplinary healthcare teams
- Maintain accurate and timely documentation
Requirements:
- Minimum 2 years of experience as a Physical Therapist
- Active California PT License
- BLS certification (AHA preferred)
- Skilled Nursing Facility (SNF) experience preferred
