Key Responsibilities- Evaluate patients and develop individualized physical therapy treatment plans within a skilled nursing facility setting
- Deliver direct patient care focused on improving mobility, strength, and functional independence
- Document patient progress, treatment sessions, and outcomes in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to coordinate patient goals and discharge planning
- Educate patients and families on therapeutic exercises and home safety strategies
Required Qualifications- Active Physical Therapist (PT) license in the state of Idaho or eligibility for licensure
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Experience working in a skilled nursing facility or post-acute care environment
Preferred Experience / Skills- Prior travel or contract therapy experience
- Familiarity with MDS documentation and SNF regulatory requirements
