Key Responsibilities- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
- Conduct 60-minute initial evaluations and 30-minute follow-up appointments, progressing patients toward discharge within an established length of stay
- Complete medical record documentation accurately and in a timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
- Design functional treatment programs that support patients' effective return to community activities
- Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned, including supervision, learning objective development, and student evaluations
- Adhere to discipline-specific state practice acts and regulatory requirements; maintain professional licensure in good standing and fulfill all continuing education requirements
Required Qualifications- Active and unrestricted Physical Therapist (PT) license in the state of Colorado
- Graduate of an accredited Physical Therapy program
- Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
- Knowledge of and adherence to discipline-specific state practice acts and professional codes of conduct
- Competency in medical record documentation in compliance with state, federal, and insurance guidelines
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting
- Experience supervising or mentoring PT students in a clinical environment
- Familiarity with performance improvement methodologies and quality initiatives
