Key Responsibilities- Evaluate and assess patients to develop individualized physical therapy treatment plans
- Deliver hands-on therapeutic interventions to improve patient mobility, strength, and function
- Document patient progress and maintain accurate clinical records in accordance with facility standards
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
- Educate patients and families on home exercise programs and injury prevention strategies
Required Qualifications- Active Physical Therapist (PT) license in the state of Florida
- Graduate of an accredited Physical Therapy program
- Experience working in a Skilled Nursing Facility (SNF) setting
- Current BLS/CPR certification
Preferred Experience / Skills- Prior travel or contract therapy experience
- Familiarity with SNF documentation and Medicare compliance requirements
