Physical Therapist (PT) - Home Health

Schedule / Job TypeContract (13 Weeks) | Full-Time | Onsite | Monday through Friday, 8:00 AM - 5:00 PM
Key Responsibilities- Provide skilled physical therapy services to patients in a home health setting
- Evaluate and treat patients within an assigned territory covering Spokane County and potentially lower Stevens County
- Develop and implement individualized treatment plans based on patient needs and goals
- Document patient progress and clinical notes using HCHB electronic medical records
- Collaborate with interdisciplinary care teams to coordinate patient care
Required Qualifications- Active Physical Therapist (PT) license in the state of Washington
- Minimum of 12 months of Home Health experience within the past 3 years
- Proficiency with HCHB electronic medical records system
- Valid driver's license and reliable transportation for patient home visits
Preferred Experience / Skills- Familiarity with Spokane County and surrounding areas
- Strong organizational and time management skills for managing a home health caseload
