Key Responsibilities- Evaluate patients and develop individualized physical therapy treatment plans within a skilled nursing facility setting
- Deliver hands-on therapeutic interventions to improve patient mobility, strength, and functional independence
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
- Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
Required Qualifications- Graduate of an accredited Physical Therapy program
- Current and valid Physical Therapist (PT) licensure in South Carolina or eligibility to obtain
- Experience working in a skilled nursing facility (SNF) setting
- Strong clinical assessment and documentation skills
Preferred Experience / Skills- Prior travel or contract therapy experience
- Familiarity with Medicare Part A and skilled nursing documentation requirements
