Key Responsibilities
  • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction
  • Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
  • Design functional treatment programs focused on progressing patients toward discharge within an established length of stay and facilitating effective return to community activities
  • Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under valid physician orders and within insurance approvals
  • Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
  • Contribute to organizational performance improvement initiatives and adhere to all clinic safety guidelines including safe patient handling, infection control, and workplace hazard reporting
Required Qualifications
  • Active and unrestricted Physical Therapist license in good standing in the state of Connecticut
  • Adherence to the state Physical Therapy Practice Act and all applicable regulatory agency requirements
  • Completion of all state-mandated continuing education requirements
  • Ability to perform in an ethical manner in accordance with discipline-specific professional codes of conduct
  • Demonstrated clinical reasoning skills to develop appropriate treatment plans and patient goals
Preferred Experience / Skills
  • Prior experience in an outpatient physical therapy setting
  • Experience supervising or mentoring physical therapy students
  • Active involvement in professional development activities, continuing education, or professional organizations
