Key Responsibilities- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
- Design functional treatment programs focused on progressing patients toward discharge within an established length of stay and facilitating effective return to community activities
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under valid physician orders and within insurance approvals
- Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
- Contribute to organizational performance improvement initiatives and adhere to all clinic safety guidelines including safe patient handling, infection control, and workplace hazard reporting
Required Qualifications- Active and unrestricted Physical Therapist license in good standing in the state of Connecticut
- Adherence to the state Physical Therapy Practice Act and all applicable regulatory agency requirements
- Completion of all state-mandated continuing education requirements
- Ability to perform in an ethical manner in accordance with discipline-specific professional codes of conduct
- Demonstrated clinical reasoning skills to develop appropriate treatment plans and patient goals
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting
- Experience supervising or mentoring physical therapy students
- Active involvement in professional development activities, continuing education, or professional organizations
