Key Responsibilities- Provide home health physical therapy services to adult patients across assigned county coverage areas
- Complete approximately 6 patient visits per day within the home health setting
- Evaluate, develop, and implement individualized physical therapy treatment plans for adult patients
- Accurately document patient care and progress using home health clinical software
- Maintain compliance with all required training modules completed through the designated learning portal
Required Qualifications- Active Maryland Physical Therapist state license required at time of submission
- Minimum 1 year of physical therapy experience required
- Home health experience required
- Current BLS certification required
- Two supervisor references required
Preferred Experience / Skills- Experience with HCHB (HomeCareBuzz) documentation software preferred
- Familiarity with Relias or Net Learning training platforms
