Key Responsibilities- Evaluate and treat patients in a skilled nursing facility setting to restore and improve physical function
- Develop and implement individualized physical therapy treatment plans based on patient needs and goals
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
- Educate patients and their families on therapeutic exercises and home safety strategies
Required Qualifications- Active Physical Therapist (PT) license in the state of Florida
- Graduate of an accredited Physical Therapy program
- Experience working in a skilled nursing facility (SNF) setting preferred
- Current BLS/CPR certification
Preferred Experience / Skills- Prior experience in a SNF or long-term care environment
- Familiarity with MDS documentation and Medicare compliance in a SNF setting
