Contract | Days | Full-Time | 5x8-Hour Shifts (8:00–17:00) | Onsite / Home Health

• Conduct skilled home health visits, completing a minimum of 6 visits per day for patients including med/surg, cardiac, ortho, neurological, post-surgical, and respiratory diagnoses
• Perform comprehensive patient assessments including OASIS documentation and ongoing clinical documentation using provided technology (laptop, MiFi, portable printer)
• Develop and implement individualized home exercise programs and provide patient and family education
• Utilize and assess equipment and devices including gait belts, walkers, wheelchairs, sliding boards, sit-to-stand lifts, and upper extremity prosthetics
• Monitor and address signs and symptoms of physical, sexual, and psychological abuse; promote safety and injury prevention
• Complete accurate billing documentation and scheduling in accordance with Medicare, Medicare Advantage, and commercial insurance requirements

• Active Florida PT license or valid compact state license required at time of submittal
• Current BLS certification required
• Minimum of 1 year of physical therapy experience
• Home health experience required
• Valid driver's license and reliable personal vehicle required for home visits
• Must be COVID-19 vaccinated or have an approved exemption on file; flu shot or declination required with mandatory masking in applicable cases

• OASIS training experience preferred
• Experience with Cerner charting preferred
• Familiarity with skilled home care documentation systems and scheduling platforms

• Work with a diverse patient population across a wide range of diagnoses in a home health setting, offering variety and clinical growth
• Technology and equipment provided, including laptop, MiFi internet access, and portable printer to support efficient field-based documentation
