Schedule / Job TypeContract | Days | Full-Time | 40 hours per week | Monday/Wednesday/Friday 7:00 AM – 4:00 PM; Tuesday/Thursday 8:00 AM – 5:00 PM | Onsite | Minimum 12 weeks, starting July
Key Responsibilities- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction.
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
- Ensure all care is provided under signed, valid physician orders and within insurance approvals as required for patient clinical care.
- Delegate and direct discipline-specific therapy services for assigned patients, designing functional treatment programs to support effective return to community activities.
- Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
- Actively pursue professional development through continuing education, self-study, and involvement in professional activities and organizations.
Required Qualifications- Active and unrestricted Physical Therapist licensure in good standing per applicable state practice act.
- Minimum of 1 year of physical therapy experience.
- Demonstrated ability to perform comprehensive patient evaluations and develop appropriate clinical treatment plans.
- Adherence to discipline-specific state practice acts, regulatory agency requirements, and professional codes of conduct.
- Completion of all state-mandated continuing education requirements.
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting.
- Experience with student supervision and clinical education programs.
- Familiarity with performance improvement methodologies and quality of care initiatives.
