Pride Health is hiring a Travel Physical Therapist to support our client’s home health services in Roaring Spring, PA (16673). This is a contract opportunity offering a 32-hour weekly guarantee and mileage reimbursement.

Job Summary:
The Physical Therapist will provide in-home care, evaluating patients and delivering individualized treatment plans to improve mobility, reduce pain, and restore functional independence.

Key Responsibilities:

  • Evaluate patient condition and develop individualized therapy plans
  • Deliver physical therapy services in accordance with the plan of care
  • Monitor progress and adjust treatment plans as needed
  • Document all visits, evaluations, and progress notes accurately
  • Educate patients and caregivers on exercises, safety, and recovery goals
  • Collaborate with physicians and interdisciplinary care teams
  • Travel to patient homes throughout Blair County

Required Qualifications:

  • Master’s Degree in Physical Therapy
  • Active Physical Therapist license (PA)
  • Current CPR certification
  • Valid driver’s license, insurance, and reliable transportation

Preferred Qualifications:

  • Home health experience
  • HCHB EMR experience

Additional Information:

  • Shift: Monday – Friday | 8:00 AM – 4:30 PM
  • Location: Roaring Spring, PA 16673 (Blair County coverage)
  • Job Type: Contract
  • Weekly Guarantee: 32 hours
  • Travel: 100% field-based | Mileage reimbursed between patient visits
  • On-Call/Weekend: Not required
  • Pay Range: $54-$56/hr. (Based on experience and qualifications)
  • Dress Code: Royal blue scrubs
