Pride Health is hiring a Travel Physical Therapist to support our client’s home health services in Roaring Spring, PA (16673). This is a contract opportunity offering a 32-hour weekly guarantee and mileage reimbursement.
Job Summary:
The Physical Therapist will provide in-home care, evaluating patients and delivering individualized treatment plans to improve mobility, reduce pain, and restore functional independence.
Key Responsibilities:
- Evaluate patient condition and develop individualized therapy plans
- Deliver physical therapy services in accordance with the plan of care
- Monitor progress and adjust treatment plans as needed
- Document all visits, evaluations, and progress notes accurately
- Educate patients and caregivers on exercises, safety, and recovery goals
- Collaborate with physicians and interdisciplinary care teams
- Travel to patient homes throughout Blair County
Required Qualifications:
- Master’s Degree in Physical Therapy
- Active Physical Therapist license (PA)
- Current CPR certification
- Valid driver’s license, insurance, and reliable transportation
Preferred Qualifications:
- Home health experience
- HCHB EMR experience

- Shift: Monday – Friday | 8:00 AM – 4:30 PM

- Pay Range: $54-$56/hr. (Based on experience and qualifications)
