Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient setting
• Evaluate, assess, and develop individualized treatment plans for patients
• Deliver hands-on therapeutic interventions in alignment with plan of care
• Complete required clinical documentation accurately and in a timely manner
• Collaborate with interdisciplinary team members to support patient outcomes

Required Qualifications

• Active Physical Therapist license in the state of Wisconsin (permanent license required)
• Prior experience in an outpatient physical therapy setting
• Ability to meet facility-specific credentialing requirements prior to start
• Willingness to complete any additional documentation required by the facility before assignment begins

Preferred Experience / Skills

• Experience working in a high-volume outpatient rehabilitation environment
• Strong documentation skills and familiarity with electronic medical records
