Schedule / Job TypeContract | Full-Time | Onsite | Day Shift | 5x8-hour shifts, 40 hours per week
Key Responsibilities- Provide direct physical therapy services to patients in an outpatient setting
- Evaluate, assess, and develop individualized treatment plans for patients
- Deliver hands-on therapeutic interventions in alignment with plan of care
- Complete required clinical documentation accurately and in a timely manner
- Collaborate with interdisciplinary team members to support patient outcomes
Required Qualifications- Active Physical Therapist license in the state of Wisconsin (permanent license required)
- Prior experience in an outpatient physical therapy setting
- Ability to meet facility-specific credentialing requirements prior to start
- Willingness to complete any additional documentation required by the facility before assignment begins
Preferred Experience / Skills- Experience working in a high-volume outpatient rehabilitation environment
- Strong documentation skills and familiarity with electronic medical records
