Physical Therapist – Home Health

Schedule / Job TypeContract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based
Key Responsibilities- Provide skilled physical therapy services to home health patients in a 1:1 care model, visiting up to 6 patients per day
- Complete OASIS assessments and documentation accurately and in a timely manner
- Chart using EPIC Remote Client for Home Health and the Rover mobile application
- Manage a daily patient caseload across a defined geographic service area, with occasional drive times up to 60–90 minutes per day
- Maintain 30 productivity points per standard shift in accordance with program expectations
Required Qualifications- Active Physical Therapist licensure in both Oregon (OR) and Washington (WA) required prior to start
- Minimum 1 year of skilled home health experience
- OASIS experience required
- Current Basic Cardiac Life Support (BCLS) certification
- Must have reliable personal transportation
Preferred Experience / Skills- Experience with EPIC Remote Client for Home Health documentation
- Familiarity with the Rover mobile charting application
- Ability to efficiently manage a geographically distributed patient caseload
