Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based
Key Responsibilities
Provide skilled physical therapy services to home health patients in a 1:1 care setting, visiting up to 6 patients per day
Complete clinical documentation using EPIC Remote Client for Home Health and Rover mobile app
Conduct and complete OASIS assessments accurately and in a timely manner
Manage a daily patient schedule across a service area, with occasional drive times up to 60–90 minutes per day
Maintain 30 productivity points per standard shift in accordance with home health guidelines
Required Qualifications
Active Physical Therapist licensure in both Oregon (OR) and Washington (WA) required
Minimum 1 year of skilled home health experience required
OASIS experience required
Current Basic Cardiac Life Support (BCLS) certification required
Must have reliable personal transportation
Preferred Experience / Skills
Experience with EPIC electronic health record system in a home health setting
Familiarity with Rover mobile documentation application
Strong time management and self-directed scheduling skills in a field-based environment
