Contract | Days | Onsite | Outpatient Setting

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction
• Maintain accurate, timely, and compliant medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
• Participate in student clinical programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
• Adhere to state Practice Act and all regulatory requirements; maintain professional licensure in good standing and fulfill continuing education requirements
• Actively engage in professional development and contribute to organizational performance improvement initiatives

• Active Physical Therapist license in good standing in the state of Arizona
• Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
• Proficiency in clinical documentation in compliance with state, federal, and payer guidelines
• Commitment to patient and employee safety standards and safe patient handling practices
• Ability to work within a team-oriented, outpatient clinical environment

• Experience in orthopedic and sports rehabilitation with a diverse patient age population
• Prior experience in an outpatient setting with a volume of approximately 10–11 patients per day
• Availability for a minimum 6-month assignment
