Key Responsibilities
Provide direct physical therapy services to patients in an outpatient or clinic-based setting
Evaluate, assess, and develop individualized treatment plans for patients
Deliver therapeutic interventions in alignment with established care protocols
Complete required clinical documentation accurately and in a timely manner
Collaborate with interdisciplinary care teams to support patient progress and outcomes
Required Qualifications
Active Physical Therapist license required; must obtain permanent Arizona licensure promptly upon assignment start
Prior experience in a physical therapy clinical setting
Ability to meet credentialing and contract quality assurance requirements
Must not have worked at this facility in a permanent or PRN capacity within the past year
Preferred Experience / Skills
Experience working in a high-volume outpatient or clinic-based physical therapy environment
Familiarity with travel or contract assignment workflows and documentation requirements
