Key Responsibilities
  • Conduct comprehensive patient evaluations (60-minute evals) and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
  • Complete all medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
  • Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
  • Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
  • Adhere to discipline-specific State Practice Act and all applicable regulatory requirements; maintain professional licensure in good standing
  • Actively pursue professional development through continuing education, self-study, and involvement in professional activities and organizations
Required Qualifications
  • Active and unrestricted Physical Therapist license in the state of Colorado
  • Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
  • Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
  • Knowledge of and adherence to discipline-specific State Practice Act and professional code of conduct
  • Commitment to patient and employee safety standards and protocols
Preferred Experience / Skills
  • Prior experience in an outpatient physical therapy setting
  • Experience supervising or mentoring physical therapy students
  • Familiarity with performance improvement methodologies and quality initiatives
