    Description

    We are seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team in a Skilled Nursing Facility (SNF). The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide evidence-based therapy services to improve mobility, strength, balance, endurance, and overall functional independence.

    As a valued member of our interdisciplinary team, you will work closely with physicians, nursing staff, occupational therapists, speech-language pathologists, and caregivers to help residents achieve their highest level of function and quality of life. The ideal candidate is patient-focused, collaborative, and committed to delivering exceptional rehabilitative care.

    Responsibilities
    • Perform comprehensive physical therapy evaluations and develop individualized treatment plans
    • Provide therapeutic exercise, gait training, balance training, transfer training, manual therapy, and functional mobility interventions
    • Assess resident progress and modify treatment plans to maximize rehabilitation outcomes
    • Educate residents, families, and caregivers on home exercise programs, mobility techniques, fall prevention, and safe transfer strategies
    • Complete accurate and timely documentation in accordance with facility policies, Medicare guidelines, and regulatory requirements
    • Collaborate with physicians, nurses, rehabilitation staff, and other healthcare professionals to coordinate patient care
    • Participate in interdisciplinary team meetings, care planning, and discharge planning
    • Maintain a safe, clean, and organized treatment environment
    • Ensure compliance with all federal, state, and facility regulations
    • Deliver compassionate, patient-centered care that promotes independence and enhances quality of life
    Requirements
    • Required: Graduate of an accredited Physical Therapy program
    • Required: Active Physical Therapist (PT) license in the state of practice or eligibility to obtain
    • Preferred: Previous experience in a Skilled Nursing Facility (SNF), post-acute care, or long-term care setting
    • Strong clinical reasoning, communication, documentation, and organizational skills
    • Ability to work independently and collaboratively within a multidisciplinary team
